Chem Tech Fumigation hereby submit specifications,estimates and offer for the control of termites at the
following location: 2106 Davis Drive,Blair,NE ;the control of termites to be undertaken in a workmanlike manner
and according to accepted practices.
Scope of service:
Exterior termiticide application to house. Probe exterior foundation soil and apply termiticide to soil directly
around foundation of house.Completing an undisturbed zone as required by product labeling.
The Proposer,hereinafter called the Exterminator,would during said period,furnish control as required. The fulfillment of any agreement would be
contingent upon the regular payments as stipulated,for services rendered. In the event of sale or other disposal of the buildings or premises described
any unpaid balance would be immediately due and payable,unless such unfulfilled obligations be assumed in writing by and between such subsequent
interest and The Exterminator. After the expiration of the prescribed term of any agreement the agreement would automatically extend for a like term
unless cancelled in writing thirty(30)days prior to the expiration date of the agreement. The Exterminator warrants to the extent of the consideration
price stated above that its control services will be satisfactory but make no other warranty,express or implied. As part of the consideration of an
agreement the Acceptor by his or its acceptance of this Proposal agrees to waive any claim he or it may have or claim to have against The Exterminator
arising out of damage to his or its person or property caused by aforementioned pests,but the Acceptor would expressly reserve the right to make claim
for any damage to persons or property caused by an negligent or careless application of control remedies by The Exterminator. No other terms,
conditions or representations would be binding unless in writing.
