If you do not return a completed Roofing Checkoff Sheet within 30 days of the,date
of this notice, you will forfeit the $50.00 deposit associated with this permit and we
will turn this issue over to the City`of Blair Licensing Board and City Attorney to
consider suspending your contractor registration.
