The Roofing Inspection for the above referenced permits have passed, but no
Roofing Checkoff Sheet has been submitted for the permits. A signed Roofing i
Checkoff Sheet is required as certification that all work was performed in
accordance with the 2009 Building Code.
A blank Roofing Checkoff Sheet is enclosed for your convenience. Please complete
the form immediately and return it by email (stormdamage@ci.blair.ne.us), by fax
(402.426.4195) or by mail (218 S 16th St, Blair, NE, 68008).
This is the second and final request for a Roofing Checkoff Sheet.
If you do not return a completed Roofing Checkoff Sheet within 30 days of the date
of this notice, you will forfeit the $50.00 deposit associated with this permit and we
will turn this issue over to the City of Blair Licensing Board and City Attorney to
consider suspending your contractor registration.
