Beginning May 24 and continuing to August 1, 2011, flooding from the Missouri River and various tributaries threatened the City of Blair's
water and wastewater treatment facilities. The sub grantee instituted emergency protective measures to protect the facilities, in uding
construction of temporary berms (ref. SAs WAFF046 and WAFF09B). Trucks carrying construction material accessed the berm from

Fairview Drive (Site 1) and one interior plant road (Site 2). Both roads were extensively damaged by the repeated heavy truck use as this
Pavement was not designed for construction traffic. Damage to these roads was not addressed in the work performed for the temporary

96.09822.) - Fairview Drive is 2,049 -ft in length, 1,703.8 -ft was damaged plus 4 plant entrances and 1 turn heavy construction traffic used to build the widening. -Two sections of
001 -ft L x 18 ft
reinforced concrete pavement were totally destroyed from he temporary berm around the water
treatment plant, (x),(1,-W x 0.5 -ft D /27) = 333.7 CY. (b), (185 -ft L x 18 - ft W x 0.5 -ft D / 27) = 61.7 CY. Aggregate surfacing
material was placed on the roadway to allow emergency work to continue and was captured on SA's WAFF04B & WAFF098. Work to be

removed, This amount, combined with pavement already removed (395.4 CY; calculations reported in Work Completed), totals 568 CY of
damaged concrete to be replaced (172.6 CY + 395.4 CY). - Connecting Fairview Drive to the facility interior roads are four (4) entrances,
each comprised of a single concrete slab (30 -ft L x 40 - ft W x 0.5 -ft D / 27 = 22.2 x 4 -ea. = 88.8 CY). All 4 entrance slabs were damaged
beyond repair and all will have to be replaced (88.8 CY). - There is a single turn - widening at one entrance (slab measuring 3 -ft L x 60 -ft W x
0.5 -ft D / 27 = 3.3 CY). This widening is damaged beyond repair and will have to be replaced (3.3 CY). - Underlying Fairview Drive, the 4
connecting entrances, and the turn - widening, is a gravel aggregate base course (0.25 -ft D). Portions of this material has saturated, settled,
and failed, and will have to be replaced. Dimensions: Fairview Drive = 284 CY (1,703.8 -ft L x 18 -ft W x 0.25 -ft D /27); entrances = 44.4 CY
(30 -ft L x 40 -ft W x 0.25 -ft D /27 x 4 ea); turn - widening = 1.7 CY (3 -ft L x 60 -ft W x 0.25 -ft D / 27). Total aggregate base course to be
replaced = 330.1 CY

Site 1: Work Completed: The damaged reinforced concrete was removed and stockpiled on site, which allowed aggregate surfacing
material to be placed on the roadway and allow emergency work to continue. The total damaged road surface removed from Fairview Drive EMA cost codes were used for = 395.4 CY. (a),(333.7 CY) @ $4.00 per CY = $1,334.80 (b), (61.7 CY

Site 2: Plant Road, 18 -ft wide paved concrete surface road (0.5 -ft D), 299 -ft long. (Start 41.55205 / - 96.09833; end 41.55273 / - 96.09863). -
Plant Road is 299 -ft in length, 188.4 -ft was damaged. - The Plant Road surface available for visual inspection (not covered by the
emergency berm) was comprised of 60 panels of reinforced concrete. It will require 62.8 CY (188.4 -ft L x 18 -ft W x 0.5 -ft D / 27) of
reinforced concrete to repair 38 broken panels (only). - Plant Road has a gravel aggregate base course (0.25 -ft D). Portions of this material
has saturated, settled, and failed, and will have to be replaced. This will require 31.4 CY (188.4 -ft L x 18 -ft W x 0.25 -ft D / 27) of aggregate
base.

Site 2: Work to be Completed: The following work will return Site 2 back to pre- disaster condition by repairing the damaged sections (only)
of the road. FEMA cost codes were used for estimating purposes in the absence of documented costs available to the sub grantee. -
Remove damaged concrete pavement (only) on Plant Road. Total to be removed = 62.8 CY @ $4.00 per CY (Code 3212) = $251.20 -
Replace, shape, and grade aggregate base course under the damaged sections of Plant Road. Total to be replaced = 31.4 CY @ $18.90
per CY (Code 3203) = $593.46 - Replace damaged concrete pavement on Plant Road. Total to be replaced = 62.8 CY @ $150.00 per CY =
$9,420.00. Site 2 Total Costs for Repairing = $10,264.66 1 recommend that the repairs to pre- disaster condition be eligible, and the
requested codes and standards upgrade be ineligible. Project Notes: 1. The sub grantee is requesting Direct Administrative Costs that are
directly chargeable to this specific project. Associated eligible work is related to administration of this PA.project only and in accordance with
44 CFR §13.22. These costs are treated consistently and uniformly as direct costs in all Federal awards and other sub grantee activities and
are not included in any approved indirect cost rates. 2. No mitigation opportunities were identified for the referenced permanent work. 3. If
any eligible portion of the scope of work described in this sub grant application is reimbursed in the future via litigation, settlement, rk any
other mechanism; FEMA is required to reduce the amount of assistance for eligible work by such amount per §312 of the Stafford Act. 4.
This a Large Project (over $63,900). Final payment will be based on documented eligible expenses. Supplemental payment will be made .
upon receipt of eligible invoices from the sub grantee. All approved sub grant applications within a project application are subject to random
audits. Quarterly reviews are conducted on all approved sub grant applications which equal or exceed $63,200 and have not received final
payment. Any jurisdiction receiving $500,000 or more in total Federal financial assistance in a fiscal year must submit a copy of its single
audit (as required by the Single Audit Act of 1984) to the state. 5. As described in CFR Section 13.42, sub grantee must maintain all work -
related records for a period of three (3) years from applicant closure (final payment). All records relative to this project worksheet are subject
to examination and audit by the State, FEMA, and the Comptroller General of the United States and must reflect work related to disaster
specific costs. 6. Document (s) entered may be subject to the Privacy Act of 1974. Document(s) entered may contain personal or privileged
information and should be treated as "For Official Use Only ". Contents shall not be disclosed, discussed, or shared with individuals unless
they have a direct need -to -know in the performance of their official duties. Unauthorized disclosure of this information may result in Personal
Liability with Civil and Criminal penalties. Complete records and cost documents for all approved work must be maintained for at least 3
years from the date the last project was completed or from receipt of final payment, whichever is later. 7. The Nebraska Department of
Environmental Quality (DEQ) has made available their requirements for meeting "State Requirements ", however, if sub grantee's are
looking for Federal Dollars for Disaster Damages, then Executive Orders 11988 and 11990 need to be reviewed at minimum. 8. Presidential
Directive 11988 Floodplain Management and 11990 Wetland Objectives are: - Avoid, to extent possible, the long - and short - term adverse
impacts associated with the occupancy and modifications of floodplains. - Avoid direct and indirect floodplain development whenever there
is a practicable alternative. 9. Executive Order 11988 requires Federal agencies to avoid the 100 -year floodplain, unless there is no
practicable alternative, or to mitigate the effects of potential flooding through such measures as elevating structures, FEMA must determine
if a project is located in a Special Flood Hazard Area (SFHA) as designated on the National Flood Insurance Program (NFIP) Maps. 10. All
documentation was reviewed /validated and found reasonable by the FEMA Project Specialist assigned. Copies of all contracts, sub
grantee's pertinent pre- disaster policies; and codes and standard upgrade requirements are attached or in the JFO. Attachments: 1. Email

12/8/1 INSURANCE CONSIDERATIONS. Applicant does not
have insurance on this facility. There is no duplication of
benefits pursuant to 44 CFR 206.250(c) and Sec. 312 of the
Stafford Act. There are no insurance proceeds to be
deducted. As detailed in FEMA Fact Sheet Number 9580.3,
the requirement to obtain and maintain insurance as a
condition of receiving public assistance funding from FEMA
applies to buildings, contents, vehicles, and equipment. The
items described in this Subgrant Application do not fall in one
of these categories. There is no insurance purchase
