Soon the payment process will begin. Once your projects are completed and payments have been
made please review and sign the Project Completion and Certification Report (PA), which you will receive
with your final federal payment. Once all Federal funds have been paid to your agency in regard to this
disaster and the P4 is returned to the Public Assistance Office State funds will be paid, if applicable. Until the

DAMAGE DESCRIPTION AND DIMENSIONS:

Beginning on May 24 and continuing to August 1, 2011, flooding from the Missouri River and various tributaries impacted the City of Blair.
The floodwaters threatened the city's water and wastewater treatment facilities and the access roads to the facilities located north of State
Highway 30 and 400 yards west of the Missouri River (access road start 41.55126 / - 96.10562). Debris and sediments were deposited in
ditches, outflow pipes, and culverts restricting drainage of water to the river. Electrical power service to the facilities was disrupted and
public access to the adjoining Optimist Park had to be restricted. Flood waters also blocked access to sites normally used for disposal of
sludge generated by treatment facilities. The coordinates listed above are for the Blair City Hall located at 218 South 16th Street, Blair, NE

Work Completed; The sub grantee used force account equipment and, using its own procurement procedures, secured contractors to
provide labor, equipment, and material to operate various pumps, clean drainage ditches and outflow pipes, and to post signage to restrict
public access. Rainwater and permitted discharge effluent that was trapped behind emergency berms (berms captured in sub grant
application (SA) WWFF09B) surrounding the treatment facilities had to be removed by pumping. Contractors were used to connect and
disconnect electrical wires to temporary flood control pumps and other facility equipment. Alternate sites for permitted disposal of facility
generated sludge were used as original sites /access was blocked by flood waters. Signage was made and posted to restrict access to
Optimist Park, which is accessed by the road to the plant and closed due to flooding. As flood waters subsided, ditches and culverts had to
be located and debris removed to restore normal drainage. The sub grantee stated that they used their normal procurement processes in
securing contract services listed in this SA. Project Notes: 1. Labor for operating the sub grantee's force equipment (a 10 horsepower pump
and associated hoses) was provided by contract labor of this SA and by contract labor covered in SAs WAFF0413, WAFF0913, and
WAFF07B (in development). Thus, no force account labor is reported in this SA. 2. The sub grantee's water and wastewater treatment
services serve more than 10,000 residential customers and over 3,300 residential customers in a rural water district, in addition to the
Village of Kennard, located in Washington county. Bulk customers served by the sub grantee include several large industrial customers
employing over 1,000 people. Cargill, Inc., which operates one of the largest corn milling operations in North America and the Omaha Public
Power District nuclear facility, both operate facilities served by the sub grantee which has a combined employment of over 4,000 people. 3.
The sub grantee is requesting direct administrative costs that are directly chargeable to this specific project. Associated eligible work is
related to administration of this PA project only and in accordance with 44 CFR §13.22. These costs are treated consistently and uniformly
as direct costs in all Federal awards and other sub grantee activities and are not included in any approved indirect cost rates. 4. Hazard
Mitigation was discussed with the sub grantee and either does not apply with this project or the sub grantee was not interested in pursuing
hazard mitigation measures. 5. If any eligible portion of the scope of work described in this Project Worksheet is reimbursed in the future via
litigation, settlement, or any other mechanism; FEMA is required to reduce the amount of assistance for eligible work by such amount per
§312 of the Stafford Act. 6. This is a Large Project (over $63,900). Final payment will be based on documented eligible expenses.
Supplemental payments will be made upon receipt of eligible invoices from the sub grantee. All approved sub grant applications within a
project application are subject to random audits. Quarterly reviews are conducted on all approved sub grant applications which equal or
exceed $63,900 and have not received final payment. Any jurisdiction receiving $500,000 or more in total Federal financial assistance in a
fiscal year must submit a copy of its single audit (as required by the Single Audit Act of 1984) to the state. As described in CFR Section
13.42, sub grantee must maintain all work - related records for a period of three (3) years from sub grantee closure (final payment). All
records relative to this project worksheet are subject to examination and audit by the State, FEMA, and the Comptroller General of the
United States and must reflect work related to disaster specific costs. Document (s) entered may be subject to the Privacy Act of 1974.

)ayment, whichever is later. 7. The Nebraska Department of Environmental Quality (DEQ) has made available the requirements for meeting
'State Requirements ", however, if sub grantee's are looking for Federal Dollars for Disaster Damages, then Executive Orders 11988 and,
11990 need to be reviewed at the minimum. Presidential Directive 11988 Floodplain Management and 11990 Wetlands Objectives are:
