DAMAGE DESCRIPTION AND DIMENSIONS:

Beginning on May 24 and continuing to August 1, 2011, flooding from the Missouri River and various tributaries impacted the city of Blair.
Due to the floodwaters, the sub grantee undertook emergency protective measures for the protection of its water treatment (WTP) and
wastewater treatment (WWTP) facilities and access road which included the construction of an earthen berm around the facilities and along
the access road. Per the design specifications, the berm was constructed on a 2.5:1 slope, faced with rip rap, anchored with concrete slurry
in sections and plastic anchored by sandbags in other sections and including silt fencing at the perimeter. The coordinates given above are
for the Blair City Hall located at 218 South 16th Street, Blair, NE. 68008. This is a draft and is subject to change. The floodwaters caused
disruption of normal gravity flows into the river from the WTP, causing the sub grantee to institute emergency pumping operations to
maintain normal capacities. The sub grantee constructed a triangular shaped rip rap retention basin east of the berms within a riverside
park. The emergency pumping resulted in the discharge of limestone sludge sediment into the basin. The floodwaters also deposited
sand /sediment and vegetative debris along the river bank and inland of the riverside park. Damage Description and Dimensions Site 1:

Marina Drive. Emergency protective berms were constructed on the north side of Fairview Drive and encompassed the sub grantee's
Water/Wastewater Treatment Plants. The total length of the berms was a measured 3,130 If, an average width at the base of 40 feet and 20
feet at the top for a total average width of 30 feet, and an average height of 8 feet . The berms were constructed of dirt, faced with 2,608 cy
of rip rap, anchored in sections with concrete slurry, plastic and 233 cy of sandbags and approximately 1,430 If of silt fencing. A separate
retention berm was constructed, starting on the east side of the main berm (Start @ Fairview Drive 41.55194 / - 96.09794 Finish 41.55331 / -
96.09831), The berm was constructed in a triangular shape entirely of rip rap and measured 462 If x 20 ft x 9 ft. The following calculations
were used to determine quantities and reasonableness: 3130 If x 30 ft (average width) x 8 ft (average height) = 751,200 / 27 = 27,822 cy
Rip Rap in the retention basin: 462 If x 20 ft (width @ base) x 9 ft (height) = 83,160 / 27 = 3080 / 2 (for shape) = 1540 cy Rip Rap on main
berm face: 3130 If x 15 ft (average width of face) x 1.5 ft (average rip rap depth) = 70,425 / 27 = 2608 cy Total rip rap: 1540 cy + 2608 cy =
4148 cy Sandbags, plastic, and slurry: 700 If x 6 ft x 1.5 ft = 233 cy Silt fencing: Approximately 1,430 If Damage Description and Dimensions
Site 2: Optimist Park, north of State Route 30, at the Missouri River (Start 41.55193 / - 96.09673 Finish 41.55311 / - 96.09656). The
floodwaters deposited 553 cy of scattered natural river sand along a 311 If x 32 ft x an average depth of 1.5 ft section of the Park's
riverfront. The sand covers one of two lanes of the Park boat ramp and the Park has remained closed since the event due -in -part to this
accumulation of sand. In the inland from the river area of the park, discharge from the emergency pumping into the retention basin
deposited 2,357 cy of scattered limestone sludge sediment in depths averaging 6 inches to a 278 If x 458 ft section. The floodwaters also
deposited 370 cy of scattered vegetative debris into this same section of the park. The following calculations were used to determine
quantities and reasonableness: Sand /sediment from the riverfront: 311 If x 32 ft x 1.5 ft (average depth) = 14,928 / 27 = 553 cy Limestone
sludgesediment from the inland area: 278 If x 458 ft x .50 (average depth) = 63,662 / 27 = 2,357 cy Total scattered sediment debris 2,357 +
553 = 2,910 cy Vegetative debris 100 If x 20 ft x 5 ft = 10,000 127 = 370 cy
SCOPE OF WORK:

Work Completed Contract - Analytical, Engineering and Testing Services: The sub grantee secured, following its own procurement policies,
the contractual services of HDR Engineering, Inc. to coordinate with the Nebraska Department of Environmental Quality (NDEQ) and
various vendors for the environmental testing and analysis of debris for hazardous materials, surveying and quantifying debris, preparing bid
documents, conduct site visits for potential bidders, and soliciting estimates from contractors for the debris removal operation. DDD -SOW
Note: Site inspections and measurements performed by the Project Specialist revealed variances in the dimensions and quantities with
dimensions and quantities contained in the bid package prepared by the sub grantees contracted engineering firm. The engineering firm's
estimate calculated less debris volumes and represent less than a 10% variance from the calculations contained in the Damage Description
and Dimensions. The Project Specialist calculations are used to determine reasonableness of the bid package. The bid package did not
address specifically the vegetative debris that was noted by the Project Specialist. This debris will be removed under the miscellaneous
items of the bid and disposed of at the same certified landfill. Contract - Bid Estimate: Per the completed bid package, the sub grantee
solicited and received, following its own procurement policies, four competitive bids for the removal and placement of approximately 21,600
cy of earthen berm into excavation; removal and stockpile of 6,000 cy of earthen berm and 2,200 tons of rock rip rap; removal and disposal
of 800 cy of river deposited sand, 1,200 cy of lime sludge, miscellaneous sandbags and plastic, and 1.430 If of silt fence to a certified
landfill, Pheasant Point (Douglas County) Landfill located at 13505 North 216th Street, Bennington, NE. 68007 (41.38144 /- 96.25338), This
Sub Grant Application (SGA) was written to the specifications of the lowest bid received for the competitive bidding process that was
received by the sub grantee from Pink Grading, Inc. for the amount of $400,000.00, which represents a unit price of approximately $12.88
per cy. All bids received were for lump sum and are attached to this SGA. Contract - Engineering Services: The sub grantees engineering
services contractor, HDR Engineering, Inc. estimates 72 billed hours of various disciplines to complete services during the debris removal
operation. These services include pre work conferences and site visits, compaction testing, change orders, and substantial and final
completion inspections. The estimate amount was derived by using the average hourly rate for the disciplines from the work completed
invoice ($111.81) multiplied by the estimated hours (72) and totals $8050.32. Project Notes: The scope of work contained in the Pink
Grading, Inc. bid references "seeding of disturbed areas ". This pertains to seeding for areas that may require slope protection and will be
performed on an as needed basis. The sub grantees procurement policy is on file at the Joint Field Office (JFO) in Lincoln, Nebraska. The
sub grantee is requesting direct administrative costs that are directly chargeable to this specific project. Associated eligible work is related to
administration of the PA project only and in accordance with 44 CFR 13.22. These costs are treated consistently and uniformly as direct
costs in all federal awards and other activities and are not included in any approved indirect cost rates. No mitigation opportunities are
available because this SGA is for emergency work. If any eligible portion of the scope of work described in this Project Worksheet is
reimbursed in the future via litigation, settlement, or any other mechanism; FEMA is required to reduce the amount of assistance for eligible
work by such amount per §312 of the Stafford Act. As described in CFR Section 13.42, the sub grantee must maintain all work - related
records for a period of three (3) years from applicant closure (final payment). All records relative to this project worksheet are subject to
examination and audit by the State, FEMA, and the Comptroller General of the United States and must reflect work related to disaster
specific costs. Document (s) entered may be subject to the Privacy Act of 1974. Document(s) entered may contain personal or privileged
information and should be treated as "For Official Use Only ". Contents shall not be disclosed, discussed, or shared with individuals unless
they have a direct need -to -know in the performance of their official duties. Unauthorized disclosure of this information may result in Personal
Liability with Civil and Criminal penalties. Complete records and cost documents for all approved work must be maintained for at least 3
years from the date the last project was completed or from receipt of final payment, whichever is later. Project Notes: The Nebraska
Department of Environmental Quality (DEQ) has made available requirements for meeting "State Requirements ", however, if Sub Grantee's
are looking for Federal Dollars for Disaster Damages, then Executive Orders 11988 and 11990 need to be reviewed at the minimum.
Presidential Directive 11988 Floodplain Management and 11990 Wetlands Objectives are: Avoid, to the extent possible, the long -and short-
term adverse impacts associated with the occupancy and modifications of floodplains. Avoid direct and indirect floodplain development
whenever there is a practicable alternative. Executive Order 11988 requires Federal agencies to avoid the 100 -year floodplain, unless there
is no practicable alternative, or to mitigate the effects of potential flooding through such measures as elevating structures. FEMA must
determine if a project is located in a Special Flood Hazard Area (SFHA) as designated on the National Flood Insurance Program (NFIP)
