electronic copy(s) of Subgrant Application(s) #216 for your agency. You have 60 days from the date of this
letter to file an appeal with the State if there are any issues regarding your Subgrant Application. This is a
copy for your records only. No further action is required at this time.
Soon the payment process will begin. Once your projects are completed and payments have been
made please review and sign the Project Completion and Certification Report (PA), which you will receive
with your final federal payment. Once all Federal funds have been paid to your agency in regard to this
disaster and the P4 is returned to the Public Assistance Office State funds will be paid, if applicable. Until the
PA is signed and returned to our office, no State disaster - related assistance payments will be made to your
agency.

Beginning on May 24 and continuing to August 1, 2011, flooding from the Missouri River and various tributaries impacted the City of Blair.
The floodwaters threatened physical damage to the city's water and wastewater treatment facilities. Surrounding the treatment facilities and
adjoining storage yards were 4060 LF X 6 FT tall industrial strength chain link fence (with 3- strand barbed wire overhang) and 250 FT of 3-
strand standard barbed wire fence (see drawing of the pre- disaster locations in Figure 1). Portions of both of these fences, panel gates, and
Posts were damaged or destroyed by rushing flood waters and the construction of an emergency berm constructed around the treatment
facilities (see SAs # WAFF04B, WAFF09B, and WAFF43B). Detailed damage description and dimensions are provided in the DDD -SOW
continuation sheets. The coordinates listed above are for the southwest corner of the chain link fence at the water and wastewater
treatment facilities, Fairview Drive, Blair, NE 68008. Item 1. Chain link fence - 1,936 LF X 6 FT high with top rail and 3 strands of barbed
wire (coordinates of southwest corner of fence are 41.55189 - 96.10240 226 LF were damaged beyond repair by rushing flood waters. 1710
LF were removed to provide space for an emergency berm. Due to urgency of building the berm, the fencing was not removed in a manner
that would facilitate reuse. Total fencing to be replaced = 1,936 LF Item 2. Chain link fence - 246 LF X 6 FT high with top rail and 3 strands
of barbed wire (coordinates of southwest corner of fence are 41.55189 - 96.10240 246 LF were damaged by rushing flood waters; this

fencing can be repaired. Site 3. 3 gates (for 6 FT high chain link fence with top rail and 3 strands of barbed wire). Gate 1. coordinates
41.55191 - 96.10205 - Double panel (9.5 LF per panel) gate was removed to provide space for an emergency berm. Due to urgency of
building the berm, one panel of two was removed in a manner that would not facilitate reuse. The other panel can be reused. Gate 2.
coordinates 41.55206 - 96.09837 - Double panel (9.5 LF per panel) gate was removed to provide space for an emergency berm. Both
panels were not damaged but need to be reinstalled. Gate 3. coordinates 41.55356 - 96.09804 - Double panel (14.5 LF per panel) gate was
pushed over by rushing flood waters. Both panels were not damaged but will need to be reinstalled. Item 4. 250 LF X 4 FT high barbed wire

SCOPE OF WORK:

Work To Be Completed: The sub grantee stated that they will use a contractor to return the fences at the water and wastewater treatment
facilities to pre- disaster condition. Cost estimates (see attached) were obtained by the sub grantee, following the City's procurement policy,
to complete work as described in the continuation sheet. Item I. Work to be Completed: Sub grantee obtained estimates (attached) from
local vendors per the City's procurement procedures to complete the following tasks: Remove 226 LF of damaged chain link fencing =
$1,243.00 Install 1936 LF (1710 LF + 226 LF) of chain link fencing = $ 27,104.00 Total cost for Item 1 = $28,347.00 Item 2. Work to be
Completed: Sub grantee obtained estimates (attached) from local vendors per the City's procurement procedures to complete the following
tasks: Repair 246 LF of chain link fencing = $ 1,476.00 Total cost for Item 2 = $1,476.00 Item 3. Work to be Completed: Sub grantee
obtained estimates (attached) from locni vo nrlors per the City's procurement procedures to complete the following tasks: Install gate 1 -
Provide one 9.5 FT gate panel = $ 829.00 - Install double panel gate = $ 800.00 Install double panel gate 2 = $800.00 Install double panel
gate 3 = $800.00 Total cost for Item 3 = $3,229.00 Item 4. Work to be Completed: Sub grantee obtained estimates (attached) from local
vendors per the City's procurement procedures to complete the following tasks: Remove 125 LF of damaged fence = $ 500.00 Install 125 LF
lof offence = $ 875.00 Repair 125 LF of fence = $ 600.00 Total cost for Item 4 = $1,975 Total estimate for all Work to be Completed =
$35,027.00 ($28,347.00 + $1,476.00 +$3,229.00 + $1,975). Project Notes: 1. The sub grantee is requesting direct administrative costs that
are directly chargeable to this specific project. Associated eligible work is related to administration of this PA project only and in accordance
with 44 CFR §13.22. These costs are treated consistently and uniformly as direct costs in all Federal awards and other sub grantee
activities and are not included in any approved indirect cost rates. 2. Hazard Mitigation was discussed with the sub grantee and either does
not apply to this project or the sub grantee was not interested in pursuing hazard mitigation measures. 3. If any eligible portion of the scope
work described in this sub grant application is reimbursed in the future via litigation, settlement, or any other mechanism; FEMA is
required to reduce the amount of assistance for eligible work by such amount per §312 of the Stafford Act. 4. As described in CFR Section
13.42, applicant must maintain all work - related records for a period of three (3) years from sub grant closure (final payment). All records
relative to this project worksheet are subject to examination and audit by the State, FEMA, and the Comptroller General of the United States
and must reflect work related to disaster specific costs. 5. Document (s) entered may be subject to the Privacy Act of 1974. Document(s)
entered may contain personal or privileged information and should be treated as "For Official Use Only ". Contents shall not be disclosed,
discussed, or shared with individuals unless they have a direct need -to -know in the performance of their official duties. Unauthorized
disclosure of this information may result in Personal Liability with Civil and Criminal penalties. Complete records and cost documents for all
approved work must be maintained for at least 3 years from the date the last project was completed or from receipt of final payment,
whichever is later. 6. All documentation was reviewed /validated and found reasonable by the FEMA Project Specialist assigned. Copies of
all contracts, sub grantee's pertinent pre- disaster policies, codes and standard upgrade requirements, invoices allowed, cancelled checks,
and samples of payroll data are attached or in the JFO. 7. The fence my be an insurable item. The sub grantee's insurance policy is on file
at the JFO in Lincoln, NE. 8. Contractor is to dispose of all fencing materials removed. Attachments: 1. Figure 1 - Line drawing of fence
