the form immediately and return it by email (stormdamage @ci.blair.ne.us), by fax
(402. 426.4195) or by mail (218 S 16th St, Blair, NE, 68008). - If you do not return a completed Roofing Checkoff Sheet within 60 days of the date
of this notice, you will forfeit the $50.00 deposit associated with this permit and we
will turn this issue over to the City of Blair Licensing Board and City Attorney to
consider suspending yourcontractor registration.
