If you do not return a completed Roofing Checkoff Sheet within 60 days of the date j
of this notice, you will forfeit the $50.00 deposit associated with this permit and we
will turn this issue over to the City of Blair Licensing Board and City Attorney to
consider suspending yourcontractor registration.
