As per our discussion yesterday, attached is the approved OPPD interconnect application for Marc Blais.

Please find attached the approved (9/15/2014) OPPD DG application for Blais, Marc — 2247 Meadow Dr., Blair, NE
(13.72kW grid -tie photovoltaic solar) received on 9/10/2014.
Please contact me (402- 552 -5837) for final OPPD verification /inspection after all equipment has been installed,
approved by local authorities, and prior to energization.
Upon final OPPD inspection and proper approvals from local authorities, the equipment may be energized and placed
into service. You may then elect 'net metering' (Rider 483) and a net meter will be installed. Please inform the electrical
inspector to forward the completed inspection /permit to OPPD's 'New Service Department', as this is needed prior to
issuing a net meter.

Consumers taking service on this Rider 483 rate schedule are not eligible for OPPD's Level Payment Plan.
