1. I, Merlyn Joyce Lutz, being first duly sworn, on oath depose and say that I
reside at 1411 North 16 Street, Perry, Dallas County, Iowa 50220.
2. That I am the surviving spouse of Robert Frank Lutz who died a resident of
Perry, Dallas County, Iowa on July 14, 1999.
3. That Robert Frank Lutz was the son of Frank Spering Lutz who died in
approximately 1943 in Blair, Nebraska.
4. That Frank Spering Lutz and his wife, Kathryn Pfaff Lutz, were the owners
of Grave Spaces 1 through 12, in Lot 1 of Block 95, Blair Cemetery located in the NE 1 /4 of
the NE' /4 of Section 12, Township 18, Range 11 in Washington County, Nebraska.
5. That Frank Spering Lutz is buried in space 12, Kathryn Pfaff Lutz is buried in
space 11, John Lutz is buried in space 1 and M.L. Lutz is buried in space 2.
6. That the surviving issue of Frank Spurling Lutz and Kathryn Pfaff Lutz are
as follows:
Daughter: Twila Lutz Larson and spouse
Granddaughter: Connie Jo Rasmussen and spouse
Granddaughter: Amy Jane Divis and spouse
Granddaughter: Dianne Kay Jahn, single
Granddaughter: Pamela Ann Gutschow and spouse
Grandson: Steven Kent Larsen and spouse
Grandson: Patrick James Lutz and spouse
Grandson: Micheal H. Lutz and spouse
Granddaughter: Linda Lou Olsen and spouse
and have each signed consents that I should be deeded Grave Spaces 3 and 4 in
Lot 1 in Block 95 of Blair Cemetery.
WHEREFORE, I request a deed be issued in the name of Merlyn Joyce Lutz for
Grave Spaces Lots 3 and 4 in Lot 1, Block 95, Blair Cemetery located in the NE' /4 of the
NE' /4 of Section 12, Township 18, Range 11 in Washington County, Nebraska.
