FIRE PROTECTION MAINTENANCE INSPECTION AGREEMENT
Associated Fire Protection agrees to furnish the necessary labor, materials, equipment, tools, supplies,
transportation and supervision for testing and maintenance according to all governmental and manufacturers
regulations.

Annual Inspection

The above fire alarm will be inspected semi-annually. The hand portable fire extinguisher inspections will be
performed on an annual basis.
Replacement parts will be billed at manufacturer's list price, less any applicable discounts. Additional labor will
be bilJed at a rate of $45.00 per man/hour regular time, $67~50 Saturday and evenjngs, and $90~OO Sunday and
holidays.
AFP will notify customers representative prior to performing any inspection, and provide this person with a copy
of the inspection report.
AFP will provide 24-hour emergency service with a maximum one....hour response time, and be on-site within two
hours~
This agreement is valid for three years from the date fisted below, Following this AFP may review pricing and/or
renew for an additional year with a 30 day written notice.
