The undersi .ed hereby requests use of the Blair City Count
Chambers by REGION 5/6 EMERGENCY MANAGEMENT
for the poses of 2011 Spring Storm. Spotter Training
For and in consideration of the use of said chambers, the
undersigned hereby specifically agrees to the following terms and
c onditions and, spec ically shall be bound. by all. provisions of this
agreement.
I. The premises abaU be used only for the poses as aped
hereinabove,
. The premises shall be used for such purpose by the undersigned.
on March 2 1 9, 2011, from 6 :00 PM to 9 :00 PM
Configuration' of the 'premises 'including placement of tables- or-
chairs shaU be restored to the condition prior the use of the
premises.
4. No alcoholic beverages or liquor shall be possess or consumed, on
the prmises and the undersigned shall take al. reasonable
precautions and actions to enforce such prohibition..
5, The undersigned shaU Sher be responsible for enforcing the
City's No SmoWngl policy in the chambers or any wart of the City
Hall premises for which utilization is permitted to the undersigned.
6. The undersigned shall be specifically responsible for locking and
sung the facilities upon terminatim of the use. No decorations
including but not limited to use of rails, tacks, staples, other
metal penetrating objects, tapes, or adhesives may be used
without prior permission from the City Administrator. Any
decorations or other display for which authorizations ions ha.s 'been
granted shall be removed, upon termination of the use by the
undersigned.,
7. The un.dersigned covenant that the 'undersigned will not use the
premises for any legal, purposes nor will the undersigned permit
same to be used for any unlawful business or pose whatsoever:
that the undersigned will not permit any nuisance whatever on
said premises; that the undersigned will instruct any quests and
ft tees as to the covenants herein and not permit any activities or
conduct by any guests or invitees which are prohibited or reclt*ed,.
herein.

authorization is in compliance with the State Fire Marshall's
