AGREEMENT

The undersigned, hereinafter referred to as "Owner," whether one or

referred to as "Fire Department."
The Owner specifically acknowledges the debris, rubble,
foundation and /or basement remaining after said structure is burned by
the Fire Department will constitute a danger and hazard. For and in
consideration of the Fire Department burning said structure, the Owner
hereby warrants and represents he has as of the date of this agreement
contracted for or has made arrangements to remove the rubble, debris,
and /or fill any excavations all of which may remain after the burning.
The Owner agrees said removal and filling will be completed not more than
ten (10) days after the burning of said structure. In the event said removal
or filling is delayed as a result of prohibitive weather conditions, the
Owner agrees to notify the Blair City Cleric of such delay.
The Owner further hereby agrees to release, indemnify, and save
harmless the Blair Volunteer Fire Department and the City of Blair from
any and all causes of action, damages, claims, suits, or any other liability of
any nature or kind arising out of or resulting from the burning of said structure
by the Blair Volunteer Fire Department.
This agreement shall be binding upon the heirs, personal
representatives, successors, and assigns of all parties hereto.

On October 18th I perfomed a visual in spection of the residence located at 1834 Lincoln Street in Blair
Nebraska. While conducting the inspection f discovered suspect: flooring that is potentially ACM. This
blue flooring was not sampled in the original survey. For this reason I cannot sign the final visual j
inpection form until bulls sample analysis confirms the material is non - asbestos containing, However, I do
certify that all of the asbestos containing materials listed in the original survey (beige linoleum), have been
removed according the the abatement scope of work.

Dear Mr. Taylor,
On August 11, 2010 RDG Geoscience & Engineering, Inc. (RDG) inspected for and
sampled presumed asbestos containing material (PACM) at your property on 1834 Lin-
coln Street in Blair, Nebraska. The residence consisted of a single family, wood frame
structure with partial earthen basement, and a detached wood frame single -car garage.
The residence, to include the garage, attic, and exterior spaces was divided into 14 func-
tional spaces for this inspection. The residence interior generally consisted of lath and j
plaster walls, plastered ceilings, and solid wood floors. Various vinyl and carpet flooring
products have been laid over the floors, but no other significant interior remodeling was
observed. The residence exterior consisted of wood lap siding, covered with aluminum
siding in places, and asphalt shingle roofing.
RDG collected 13 PACM composite samples and submitted them for laboratory analysis
by Polarized Light Microscopy (PLM). Laboratory analysis indicated one material, the
tan linoleum located beneath the kitchen carpet, is an asbestos - containing product.
RDG did not pull up enough kitchen carpet to determine the precise quantity of tan lino-
leum present, but the potential square footage does not exceed 214 square feet.
A list of the PACM materials is included in Table 1 of this report. A building floor plan
with sampling locations and analytical results are appended to this report as well.

Analysis and Method \
Summary of polarizing light microscopy (PLM / Stereomicroscopy bulk asbestos analysis) using the methods described in 40CFR Part 763
Appendix E to Subpart E (Interim and EPA 600 / R -93 / 116 (Improved). The sample is first viewed with the aid of stereomicroscopy. Numerous
liquid slide preparations are created for analysis under the polarized microscope where identifications and quantifications are preformed. Calibrated
liquid refractive oils are used as liquid mouting medium. These oils are used for identification (dispersion staining). A calibrated visual estimation is
reported, should any asbestiform mineral be present. Other techniques such as acid washing are used in conjugation with refractive oils for
detection of smaller quantities of asbestos. All asbestos percentages are based on calibrated visual estimation traceable to NIST standards for
regulated of asbestos. Traceability to measurement and calibration is achieved by using known amounts and types of asbestos from standards
where analyst and laboratory accuracy are measured. As little as 0.001% asbestos can be detected in favorable samples, while detection in
unfavorable samples may approach the detection limit of 0.50% (well above the laboratory definition of trace).
Discussion
Vermiculite containing samples may have trace amounts of actinolite - tremolite, where not found be PLM should be analyzed using TEM methods
and / or water separation techniques. Suspected actinolite /vermiculite presence will be indicated through the sample comment section of this report.

Fibrous talc containing samples may even contain a related asbestos fiber known as anthophyllite. Under certain conditions the same fiber may
actually contain both talc and anthophyllite (a phenomenon called intergrowth). Again, TEM detection methods are recommended. CA Labs PLM
report comments will denote suspected amounts of asbestiform anthophyllite with talc, where further analysis is recommended.
Some samples (floor tiles, surfacings, etc.) may contain fibers too small to be delectable by PLM analysis and should be analyzed by TEM bulk
protocols.

A "trace asbestos" will be reported if the analyst observes far less than 1 % asbestos. CA Labs defines "trace asbestos" as a few fibers detected
by the analyst in several preparations and will indicate as such under these circumstances.
Quantification of <11% will actually be reported as < =1% (allowable variance close to 1% is high). Such results are ideal for point counting, and the
technique is mandatory for friable samples (NESHAP, Nov. 1990 and clarification letter 8 May 1991) under 1 % percent asbestos and the "trace
asbestos ". In order to make all initial PLM reports issued from CA Labs NESHAP compliant, all <1% asbestos results (except floor tiles) will
be point counted at no additional charge.

Qualifications
CA Labs is accredited by NVLAP and AIHA for selected test methods for bulk asbestos fiber analysis (PLM) and airborne fiber analysis (TEM). All
analysts have a college degree in a natural science (geology, biology, or environmental science) or are recognized by a state professional board in
one these disciplines .Extensive in -house training programs are used to augment education background of the analyst. The group leader of polarized
