Project #C08341 Dear Mr. Buchardt: On September 8 2008, the residence located at 2215 Hollow Road in Blair, Nebraska was inspected for asbestos- containing building materials by Mr. Jerod Frost of AMI Environmental, Mr. Frost has completed the requisite training for asbestos accreditation as an inspector at a state approved training provider under the Toxic Substance Control Act (TSCA Title II). Mr. Frost's United States Environmental Protection Agency (EPA) and State of Nebraska Building Inspector numbers are 7ME08070802ANIR002 and 932, respectively. Inspection of the residence was performed in accordance with USEPA National Emission Standard for Hazardous Air Pollutants (NESHAP) regulations (ref.: 40 CFR, Part 61). Survey criteria included a pre - demolition inspection of the structure to identify suspect asbestos - containing building materials, which may be impacted by future demolition activities. Friability was determined by touching and / or sampling the material. 2.0 Sample Analysis A total of ten bulk samples were collected from the structure. Suspect materials included: floor tile, plaster system, window caulk, mastics and shingles with felt paper. Polarized Light Microscopy analysis, utilizing dispersion staining techniques (ref.: EPA Method 600/R- 93/116), was performed on fifteen heterogeneous applications. \rei \1 ?1J.'ik1`61C 11v r I "E 117 Ijl ,c_,oy`tl 8802 South 135th Street, Suite 100 • Omaha, NE 68138 -6511 800.828.8487 • 402.397.5001 • 402.397.3313 (fax) NESHAP Asbestos Containing Materials Survey Residence 2215 Hollow Road, Blair, NE AMI Project #: C08341 2 of Suspect asbestos- containing materials sampled at the time of the survey are outlined in the following table. EPA's NESHAP, 40 CFR, Subpart M, 61.145, requires an owner of a facility to submit proper notification with either the EPA's regional office and/or the state and local regulatory agency prior to renovation or demolition. 40 CFR 61.145 Standard for Demolition and Renovation (b) (1) Provide the Administrator with written notice of intention to demolish or renovate. Notifications must be received by the appropriate regulatory agencies 10 working days prior to commencement of site work. If demolition or renovation date changes, or the scope of work is increased by more than 20 percent, then another notification must be made. (b) (3) (i) At least 10 working days before asbestos stripping or removal work or any other activity begins, if the operation is described in paragraphs (a) (1) and (4) of this section. If the operation is as described in paragraph (a) (2) of this section, notification is required 10 working days before demolition begins. NESHAP Asbestos Containing Materials Survey Residence 2215 Hollow Road, Blair, NE AMI Project #: C08341 3of3 B. Asbestos Renovation and Demolition Issues Respectfully submitted, Thomas Manion Senior Project Manager In accordance with NESHAP's requirement 40 CFR 61.145 Standard for Demolition and Renovation, the building owner is required to do the following: (c) Procedures for asbestos emission control: (1) Remove all Regulated Asbestos Containing Materials (RACM) from a facility being demolished or renovated before any activity begins that would break up, dislodge, or similarly disturb the material or preclude access to the material for subsequent removal. Local and state agencies may also take a very active role in ensuring that all RACMs are removed prior to demolition. The work should be done by a licensed abatement contractor who should follow work practices, worker protection and disposal requirements set forth in 29 CFR 1910.1001, 29 CFR 1926.1101, 40 CFR 763, as well as contract specifications. Both agencies agree that all friable and non-friable materials (Category I & II) should be removed prior to demolition. Both agencies also concur that mastics and adhesives can be left in the facility during demolition if: Attachment: Analytical Results (c) Procedures for asbestos emission control: (1) Need not be removed before demolition if (iv) They are Category I non friable ACM and the probability is low that the materials will become crumbled, pulverized, or reduced to powder during demolition. During demolition of the facility, the demolition contractor must keep the structural components wet if they are covered with the RACM, if not removed. (c) Procedures for asbestos emission control: (2) When a facility component that contains, is covered with, or is coated with RACM is being taken out of the facility as a unit or in section: (i) Adequately wet all RACM exposed during cutting or disjoining operation. All analysts have received the necessary in-house and extramural training McCrone Research and/or University Degree in Geology, Biology, Environmental and Material Science) to perform analysis of bulk samples for the presence or absence of asbestos. Greater than one percent are re- examined by a second analyst for intralaboratory quality control. Greater than one percent are re- examined by the same analyst for quality control. All analysts are required to participate in quality control analysis rounds. Microscopic calibrations are performed on a daily, weekly and monthly basis. CA Labs is accredited by the National Voluntary Laboratory Accreditation Program ( NVLAP) for selected test methods for bulk asbestos fiber analysis (PLM) and airborne fiber analysis (TEM). Traceability to measurement and calibration is achieved by using known amounts and types of asbestos from standards where analyst and laboratory accuracy are measured. Some samples (floor tiles, surfacing, etc.) may contain fibers too small to be detectable by PLM. All asbestos qualification is traceable to NIST standards for regulated asbestos types.
