41 2010 and the Release of Notice of Violation and Property Maintenance Agreement and map filed in Book
560, pages 604-605 on October 8, 2010 for the property legally described as:
Lot 10 and Part of Vacated Street, Block 90, Blair City, Washington County, Blair, Nebraska
The physical address of the real estate is 1130 Park Street, Blair, Nebraska. The City of Blair hereby authorizes
the Washington County Register of Deeds Office to release the above-described Notice of Violation,
Agreement and Release of Notice of Violation and Property Maintenance Agreement and map on said real
estate in that said violation has been satisfactorily completed and removes any previous "Notice to Future
Buyers" statement.

This agreement is entered into on June 1, 2010 by and between Jim Blazina, hereinafter
referred to as "Blazing" and the City of Blair, Nebraska, hereinafter referred to as "City".
LEGAL DESCRIPTION: Lot 10 & Part of Vacated Park Street, Block 90, Blair City, Washington
County, Nebraska.
ADDRESS: 1130 Park Street
Whereas, Blazina has been presented with and is in acknowledgement of the property
maintenance report dated May 5, 2010 that is attached to this agreement regarding the above
property; and
Whereas, this agreement shall be filed with the Washington County Register of Deeds
Office and be made part of the permanent record relating to this property; and
Whereas, Blazina agrees to work with the City after he obtains possession of the
property to bring the existing residential structure into compliance with the 2009 International
Property Maintenance Code on or before July 1, 2010; and a
Whereas, Blazina further agrees to obtain any required permits for this project; and
Whereas, Blazina further understands that as part of the permit process he must
register with the City as a "General Contractor" or enter into an agreement with a "General
Contractor" that is registered with the City, and that all subcontractors performing work
involving plumbing, electrical and mechanical systems shall be registered or licensed by the
City, as required, and that all required permits shall be obtained.
Now therefore it is agreed by all parties that:
1. The City will execute a release document, filed with the Washington County Register of
Deeds office, relating to this property.
2. The following work shall be performed by licensed contractors and with all required
permits:
a. Cleanup of basement junk.
b. Grass, weeds, brush and limbs removed.
c. Garage structurally repaired or torn down. City Building Inspector must approve any
structural repair plans.
d. Window frames and panes repaired or windows replaced.
e. Porch cover columns replaced.
f. Basement stairs must meet current code with all required guards and handrails.
g. Provide backdraft damper for clothes dryer exhaust duct.

h. Plumbing system maintained in working order and kept free from leaks.
L HVAC system inspected by a licensed mechanical contractor and any necessary repairs
completed.
j. Remove insulation lying on furnace vent.
k. Electrical system must meet current code.
I. Smoke alarms must meet current code, including the requirement that they be hard
wired and interconnected.
3. That all work shall be completed by July 1, 2010.
4. No occupancy, transfer and/or change of ownership shall be granted until a Final Inspection
is approved by the City Building inspector, verifying that all repairs and construction
required by this Agreement have been completed per the approved plans and
specifications.
This Agreement shall be binding upon and inure to the benefit of the parties hereto,
their successors and assigns.
