telephone number, and mailing address of the local franchising authority on
monthly subscriber bills unless the franchising authority requests in writing that
the cable operator omit the information. (47 C.F.R. § 76.952) Cox
Communications currently includes the information on the bills it sends its
customers in your community. If you would like Cox to discontinue this practice,
please initial the appropriate option where indicated below. Cox will implement
the change at the earliest possible time.
Additionally, please confirm below that the name and address of the local
franchising authority that we currently list in our database is correct. This contact
information will be used either on monthly subscriber bills should you elect below
OR be given to customers who request it, should you elect below to remove the
address from monthly subscriber bills.
