The Washington County FSA Office has completed a farm combination resulting in the attached farm(s).
Attached for your information is a Base and Yield Notification, FSA-156EZ, and FSA map(s) of the farm(s). The
Base and Yield Notification shows the base acres, program yields, and Conservation Reserve Program (CRP)
reductions (if applicable) associated with the farm(s) and each specific tract.
The FSA-156EZ provides an overview of all FSA program acreage and ARC -PLC program election data
applicable to the farm(s). The map identifies all field boundaries, acreage, and NRCS determinations regarding
whether fields are highly erodible or have associated wetland determinations.
Due to the reconstitution of this farm, the ARC/PLC contract has been cancelled. Producers with interest
in the contract need to sign a new contract within 30 days. The contract will be sent in a separate
correspondence.
Acreage Reporting Requirements:
Contact this office to ensure acreage reporting requirements have been met for the resulting farm(s). All acres,
including non -cropland forage, must be reported to maintain eligibility for various FSA program benefits.
You are reminded that this reconstitution will change your farming operation and may require you to update the
Highly Erodible Land Conservation (HELL) and Wetland Conservation (WC) Certification (AD -1026) with this
office. Please note, if this reconstitution resulted in the combination or division of existing cropland fields, a new
highly erodible determination has been requested from the local NRCS. NRCS will provide additional notification
of all new determinations once completed.
If you believe the results of this farm reconstitution are in error, you may appeal to the County Committee at the
above address within 30 calendar days after you receive notice of this letter. If you do not appeal timely to the
County Committee, these determinations will become final and no further consideration on the matter will be
given.

As a result of a reconstitution, this newly created farm, as of the date of this notice, has not been enrolled in any contract or program
application. In order for the base acres and the farm to be eligible for any program benefit (if available), the producers on this farm must act
to enroll or apply in accordance with the rules governing that program by the later of 30 days from the date of this notice or the end of the
enrollment or application period if an application period exists or is in effect on the date this notice is issued. Contact your FSA service
center office for details.

This notice is issued by the county FSA office. You may appeal the accuracy of information contained in this notice to the County
Committee by filing a written request within 30 calendar days after you receive this notice in accordance with the FSA appeal procedures
found at 7 CFR Part 780. If you appeal to the County Committee, you may later appeal an adverse determination of the County Committee
to the FSA State Committee or the National Appeals Division or request mediation. To appeal, write to the County Committee at your FSA
service center address and explain why you believe this notice is erroneous. The final date to appeal this data to the county FSA committee
is the appeal date listed above.
