Yes, the city will need to sign them, and the local Authority Having Jurisdiction (AHJ), then get the forms back to us and
we will fill in the dealer line. You or someone in your organization will need to get the AHJ's signature as you will be able
to articulate the reason for the exception to the fire code to the AHJ.
It's not necessarily the state fire marshal that is the AHJ in your area as Blair requires permits. Whomever does
permitting, inspections, and signs off on new fire systems is likely it. It could also be the fire chief that is the AHJ. The
company that does your annual fire inspections should be able to tell you as well.
I do apologize for not having that information, but HunTel doesn't do fire alarm installation or inspections so we don't
know who the AHJ would be in Blair. We are a security system company that does the monitoring of many fire systems,
as a number of fire installation/inspection companies don't do their own monitoring.

So the City needs to sign these forms and get the SFM to sign off on it before I return it or does
the city sign it and then you obtain that signature. I am not sure who the dealer is that should
sign. If you could clarify, it would be greatly appreciated. Thank you.

Attached are the forms for each location that we require in order to have the premises and call list gone through prior to
dispatch. It's up to the fire marshal that covers Blair (AHJ) if that change can be done or not. If the dispatch orders need
to be amended to only the premises, or premises and 1 contact, ect. let me know and I can adjust the forms accordingly
and re send them out.

RE: Accounts for the City of Blair
Dear Sir or Madam,
The City of Blair uses Huntel Security for monitoring services at the Blair Public Library &
Technology Center, 2233 Civic Dr, Blair, and Blair City Hall, 218 S. 16th St., Blair.
I need to make a change to the order of calls when a fire alarm goes off at both locations. The
person in charge of this retired and no one knows the 4 -digit code. I was told by someone on the
phone that I needed to submit a letter to info (&huntelsecurity.com submitting account numbers.
Unfortunately, we can find no copy of the agreement between us and your company. Therefore, I
have no idea what the account numbers are.
Attached you will find a copy of the last two invoices that we paid. Hopefully, this will provide
enough proof to talk to me about updating our call list as well as a new pin number that we can now
put in a safe place where more than one person knows it.
