2021-01-12 Work Order #4 Olsson Septic System RelocationMASTER AGREEMENT WORK ORDER WORK ORDER NO. 4 - SEPTIC SYSTEM RELCOATION This exhibit dated January 12, 2021 is hereby attached to and made a part of the Master Agreement for Professional Services dated October 15, 2020 between Blair Airport Authority ("Client") and Olsson, Inc. ("Olsson") providing for professional services. Olsson's Scope of Services for the Agreement is as indicated below. GENERAL Olsson has acquainted itself with the information provided by Client relative to the project and based upon such information offers to provide the services described below for the project. Client warrants that it is either the legal owner of the property to be improved by this Project or that Client is acting as the duly authorized agent of the legal owner of such property. PROJECT DESCRIPTION AND LOCATION Project will be located at: Blair Municipal Airport, Blair, Nebraska Project Description: Work Order No. 4 — Septic System Relocation SCOPE OF SERVICES Olsson shall provide the following services (Scope of Services) to Client for the Project: Task 1: Site Visit — Evaluation of extension of existing septic system location Conduct site visit and on-site meeting to discuss scope of project. Establish requirements for permitting for the relocation of the existing septic tank and drain field per the County and State requirements for the Corporate Hangar area north of the existing drainage Swale. Task 2: Field Topographic Survey — existing and extension of septic system location Complete field topographic survey of the proposed location of the drain field between the apron and parallel taxiway. Field survey also included elevations on the existing septic tank and sanitary sewer line as exposed by City personnel. Task 3: Engineering Evaluation - existing and future usage of septic system Evaluate current usage, layout, and sizing of the existing septic tank system. Determine future usage, proposed layout of sanitary service, and prepare a spreadsheet to analyze the potential service needs for full development with due consideration given to topography, established hangar layout per the Airport Layout Plan. Septic analysis will be based on servicing the corporate hangars to the north of the existing drainage swale. Future construction of hangars to the south of the drainage Swale will be serviced by a separate septic tank and drainfield to be installed a later date. 601 P Street / Suite 200 / Lincoln, NE 68508 0 402.474.63111 olsson.com Task 4: State Permit Submittal Submit and coordinate permit submittal for onsite wastewater treatment system (per Title 124 "Authorization by Rule < 1000 gpd") Task 5: Deliverables Exhibit with proposed layout of sanitary service to be included with permit submittal. Excel spreadsheet depicting the usage flows (current and future assumed) utilized for the project design. Spreadsheet will be designed to allow for Airport Authority to input any modifications to system as Corporate Hangar area north of the drainage swale expands. Understood Owner Responsibilities: Coordination and collection of results for soil percolation tests required for permit submittal. Bidding and coordinating Contractor to install project. Closure of existing septic tank given NDEE requirements (see attached Closure Requirements) Registering proposed septic tank system (see attached Onsite Registration fact sheet) Should Client request work in addition to the Scope of Services, Olsson shall invoice Client for such additional services (Optional Additional Services) at the standard hourly billing labor rate charged for those employees actually performing the work, plus reimbursable expenses if any. Olsson shall not commence work on Optional Additional Services without Client's prior written approval. Olsson agrees to provide all of its services in a timely, competent and professional manner, in accordance with applicable standards of care, for projects of similar geographic location, quality and scope. SCHEDULE FOR OLSSON'S SERVICES Unless otherwise agreed, Olsson expects to perform its services under the Agreement as follows: Anticipated Start Date: January 12, 2021 Anticipated Completion Date: February 26, 2021 Olsson will endeavor to start its services on the Anticipated Start Date and to complete its services on the Anticipated Completion Date. However, the Anticipated Start Date, the Anticipated Completion Date, and any milestone dates are approximate only, and Olsson reserves the right to adjust its schedule and any or all of those dates at its sole discretion, for any reason, including, but not limited to, delays caused by Client or delays caused by third parties. Page 2 of 3 COMPENSATION Client shall pay to Olsson for the performance of the Scope of Services, the actual time of personnel performing such services in accordance with the Labor Billing Rate Schedule(s), and all actual reimbursable expenses in accordance with the Reimbursable Expense Schedule attached to this agreement. Olsson shall submit invoices on a monthly basis, and payment is due within 30 calendar days of invoice date. Olsson's Scope of Services will be provided on a time -and -expense basis not to exceed $7,700.00. TERMS AND CONDITIONS OF SERVICE We have discussed with you the risks, rewards and benefits of the Project, the Scope of Services, and our fees for such services and the Agreement represents the entire understanding between Client and Olsson with respect to the Project. The Agreement may only be modified in writing signed by both parties. Client's designated Project Representative shall be Rod Storm and Geary Combs. If this Work Order satisfactorily sets forth your understanding of our agreement, please sign in the space provided below. If you believe that this guidance document imposes additional requirements or penalties on regulated parties, you may request a review of the document. 12-039 Closure of Septic Tank and Lagoon Systems Fact Sheet November, 2016 Title 124 - Rules and Regulations for the Desiqn, Operation, and Maintenance of Onsite Wastewater Treatment Systems requires that whenever the use of an onsite wastewater treatment system is discontinued following connection to a sanitary sewer or following condemnation or demolition of a building or property or due to the construction of another onsite wastewater treatment system, the system must be properly closed. In addition, any further use of the onsite wastewater treatment system for any purpose is prohibited. One of the following two methods must be used for the closure of a septic tank or holding tank: • Pump and Fill Method - The tank is pumped of all liquids and solids and then filled with soil. The soil must be tamped completely so as to prevent voids which would occur as a result of settling; or • Pump and Remove Method - The tank is removed after being pumped of all liquids and solids and the void left from the tank removal is filled in with soil. The soil must be mounded to provide for future settling. Note: Disinfectant or anti -bacterial products can be used to clean the tank as an optional step in preparing the tank for closure. The following must be used for closure of a wastewater lagoon: • The lagoon is drained or allowed to evaporate until there is no liquid remaining; • The fence is removed and the settled solids and liner material at the bottom of the lagoon is scraped out and properly disposed; and • The lagoon area is leveled and filled with clean soil. The soil must be mounded over the lagoon area to provide for future settling and to prevent water from ponding. Only a certified professional, a professional engineer, a registered environmental health specialist or a person under their direct supervision may engage in the closure of an onsite wastewater treatment system. A guidance document does not include internal procedural documents that only affect the internal operations of Phe agency and does not impose additional requirements or penalties on regulated parties orinclude confidential information or rules and regulations made in accordance with the Administrative Procedure Act. ifyou believe that this guidance document imposes additional requirements or penalties on regulated parties, you may request a review of the document, 10-002 October, 2016 Onsite System Registration Fact Sheet Any onsite wastewater treatment system constructed, reconstructed, altered, modified, or otherwise changed by a certified professional, professional engineer, or registered environmental health specialist must be registered with the Department by the certified professional, professional engineer, or registered environmental health specialist within 45 days of completion of the construction, reconstruction, alteration, modification, or other change. The Department has interpreted completion of work to mean that all onsite wastewater treatment system components have been installed, constructed or modified so that the system piping, treatment devices or other appurtenances that convey, treat or dispose of wastewater are ready to be used. The NDEQ does not consider construction completion of an onsite system to be dependent upon construction of any dwelling or building to which the system may be connected, final grading, seeding, or the date of receipt of payment for work performed. Registration is not required for inspecting, pumping, or performing a repair to a system. Title 124 — Rules and Regulations for the Design, Operation and Maintenance of Onsite Wastewater Treatment Systems defines repair as the correction of a mechanical, electrical, or minor structural defect in an existing onsite wastewater system component such as, but not limited to, sealing a crack in a tank lid, repairing or replacing a tank baffle or access manhole riser, repairing or replacing a pump or electrical switch, leveling a distribution box, replacing a building sewer pipe, or replacing a cracked pipe between the septic tank and soil absorption system. Repair does not include replacement, reconstruction or modification of a tank or soil absorption system; extension or enlargement of a soil absorption component and system; replacement of a distribution pipe; or repair or replacement of a metal or concrete block tank. To register an onsite wastewater treatment system, the certified professional, professional engineer, or registered environmental health specialist must submit the required system information with the non-refundable registration fee of $140. An initial late registration fee is required for any registration received by the Department 46 to 90 days after completion of the system and is in addition to the system registration fee. A final late registration fee is required for any registration received by the Department 91 or more days after completion of the system and is in addition to the system registration fee.
