District #1 of Blair governing the use of school facilities. It is hereby agreed that the
organization or party contracting for the use of school facilities of School District
No. 1 of Blair, assumes full responsibility for damage to property or injuries to
persons or other liability that may arise in connection with said leasing on the dates
specified in this contract and will hold harmless said School District from any and
all liability because of said damage to property or injuries to persons.

schools facilities Blair High School Auxiliary Gym to the City of Blair/Blair Alumni
Volleyball Organization on the following dates and for the following purposes to the

Purpose: Alumni Volleyball Tournament

FACILITIES USE AGREEMENT
Angels Share, Inc DANA CAMPUS
Angles Share, Inc (herein referred to as the "Owner") agrees to provide facilities on the Dana campus for an
activity called basketball: put on bythe organization Blair Basketball Club and the City of Blair (herein
referred to as the "Sponsor") on the following date(s):
April 28 & April 29, 2018 between the hours of 0700 and 1700
The Owner will provide electricity, water, bathrooms and heat/cooling as reasonable for the intended activity
in the facility. The Owner will not be responsible for providing any supervision or staff for Sponsor's activity.
Sponsor is responsible for inspecting the facility and determining its fitness for the intended activity.
The SPONSOR, in turn, agrees to pay for the facilities used pursuant to this agreement, and for any
subsequently requested facilities, at the rates indicated herein or quoted at the time of request, If said rates
are not included in this document.

SUPERVISION
Youth groups, with members 18 years of age and under, shall be at all times, under direct supervision of an
individual 21 years of age or older. At least one adult supervisor is required for every 15 youths to properly
supervise. The appointed supervisor agrees to take full responsibility for the supervision of the youths and
patrons while using the facilities on the Dana campus. To avoid personal injury or damage to the facility,
participants are strictly prohibited from roaming the facility halls/rooms that are not required for their event,
Violation of this will result in the SPONSOR and/or participant being asked to leave.
The Owner retains the right to require any participant of the activity to leave the Dana cam pus premises if
the Owner or Owner's staff, in their sole and exclusive discretion, feels that circumstances require it.
EVENT CLEAN-UP
Upon completion of your activity, the SPONSOR is required to clean up any trash brought onto the facility
by Sponsor or its participants. In addition, it is not required but highly recommended to take additional
steps to keep the areas clean including sweeping and mopping the gym floors.
DAMAGES
The SPONSOR agrees to pay for all damages to facilities caused by participants not due to normal usage.
PROOF OFINSURANCE
Prior to the scheduled start date, proof of satisfactory liability insurance for the activity must be received by
the Owner no later than ten working days prior to the scheduled start date. Required minimum amounts are
$500,000 per claim and $1, 000,000 overall.
The Owner does not assume any liability for loss, damage and/or stolen personal property items.
PAYMENT
Rased on the above facility use services the estimated bill is $800.00. A $100,00 deposit must be remitted to
the Owner with this signed agreement. The activity Sponsor will provide accurate participant counts and
usage hours to Owner at least monthly. The Owner will submit final bills for facilities use monthly. All
accounts must be paid within ten working days of the billing date. Facilities use payments are to be
addressed to:

AGREEMENT TERMINATION
Owner has the right to terminate this agreement and prohibit the SPONSOR from further access to the Dana
facilities, The Owner will provide written notice to the SPONSOR upon termination ofthe agreement
CANCELLATIONS
Cancellations must be made working days prior to the scheduled facilities use, Any cancellations
after that date will result in a forfeiture of your deposit.
BUILDING ACCESS &SECURITY
The Owner will provide access to the SPONSOR for the activity via key access. All SPONSOR representatives
agree that only properly authorized individuals may access the Dana campus facility(s) for the above designated
activity, The SPONSOR will not allow participants access to any Dana campus facility(s) for their own private use and
will restrict usage to the designated activity.
If keys are issued to any SPONSOR representative, it is the policy of the Owner to charge
$50. Doloperating lock for each lost key not returned at the conclusion of the facility usage. This charge
represents actual cost of labor and parts for rekeying a door.
ALCOHOL
it is the Owner's policy, that alcohol may not be sold or consumed on Dana Campus premises except with express
written approval. This agreement does NOT provide permission in any circumstances for alcohol to be brought to, sold
on or consumed on Dana Campus by SPONSOR representatives or participants.
WAIVER OF LIABILITY
The Owner shall not be liable or responsible in any way for, and the SPONSOR, hereby waives all claims against the
Owner with respect to or arising out of, any death or injury of any nature whatsoever that may be suffered or sustained
by SPONSOR, its employees, invitees, participants, or agents of the SPONSOR or any other person as a result of any
SPONSOR actions, directly or indirectly, or any loss or damage or injury to or theft or loss of any property belong Ing to
the SPONSOR or any employees, invitees, participants, or agents of the SPONSOR on the Dana campus premises. In
addition, participants of the activity put on by the SPONSOR do hereby freely and voluntafily waive, release and
forever discharge the Owner, its directors, officers, agents, representatives or employees, and all cooperating
businesses and organizations from all liability, claims of damages, demands, actions, illnesses, death and causes
whatsoever in any manner relating to, arising from, or growing out of, or in any way connected with their participation in
this activity. The provisions of this liability waiver survive the termination of this agreement with respectto any damage,
injury, illness, or death occurring prior to such termination of this agreement.
INDEMNIFICATION
The SPONSOR has inspected the facility and is satisfied with its condition for Sponsor's activity and agrees to
indemnify and hold harmless the Owner and its directors, officers, agents, representatives and employees against any
and all claims, lawsuits, settlements, judgments, costs, penalties and expenses, including actual attorneys' fees, in
whole or in part resulting from, arising from, or in any way from the use of the facility for Sponsor's activity and/or
connected with the acts, errors or omissions, including the dishonest, fraudulent or criminal acts of the SPONSOR or its
representatives or employees, whether acting alone or in collusion with others in connection with the performance of
their obligations underthis agreement.
The undersigned individual is authorized to enter into this agreement on behalf of the SPONSOR.

information intended only for the use of the entity named as the recipient Sponsor.
